Crestview Manor's Social Service
Director for the past 11 years has been Sandi Pyles, LSW. Sandi has a
Bachelor's Degree in Social Work from Glenville State College. She
has worked in the medical field for over 20 years. As the social Service
Director, Sandi, provides many services, such as:

Taking Referrals from health care facilities, community
agencies, and general public. Plus offering information concerning
availability
of beds and offering general information about nursing home placement.

Coordinate the admission into the nursing home
by assisting the resident and their family with the transition from
home to the nursing
home setting.

Provide information and assist with the execution
of an Advance Directive(s) for residents, family members, or general
public.

Arranging and submitting referrals to outside
agencies to benefit the resident or family.

Providing information about Medicare, Medicaid
and VA.

Assist with discharge planning.

Arrange and assist with counseling for residents.
Within or outside the facility.
-
Being a part of the interdisciplinary care
plan team and promoting quality care for residents and meeting their
psycho social needs.
